During the walk through of the playground, there were three children inside a gate around the wall while the teacher was tending to a child that did not want to leave the playground. *Operation put a carabiner on the gate to not let it open*

A caregiver did not know the number of children in their care after combining classes for nap time.

This standard was found deficient as part of a DFPS Investigation.

During the investigation process, it was determined that the infant classroom was out of ratio when a caregiver left the room leaving one caregiver with five infants under twelve months old.

During the investigation process, a caregiver did not know the number of children that were in their care at drop off time.

During the investigation process, a caregiver did not maintain supervision of a child. The child was able to leave the classroom, go through another classroom, open the door to the playground and enter the playground unattended.

During the investigation process, a caregiver did not follow the operational procedures for doing hourly name to face checks, did not have the name to face checks verified by another staff member, and did not follow the protocols set in place when the operation is short staffed.

Caregivers did not use good judgment when they did not contact EMS after finding a 3 year old child on the playground with no jacket. The child had been on unattended on the playground for over 2 hours. Temperatures that day ranged from 23 degrees to 34 degrees Fahrenheit

The operation did not notify the parents immediately about their child being left on the playground. The parent was notified several hours after the incident occurred.

Two caregivers did not have administering medication training at the time of review.

An infants hands were not washed after their diaper was changed.

At time of inspection, the school aged class was observed to have cleaning solutions at childrens reach. THis was corrected at inspection.

It is found though investigation evidence a staff was assigned a classroom when they did not have the skills to safely supervise the entire group in care.

The loose fill under 2 slides is not 6 inches deep during the inspection. note: This was corrected during the inspection when the loose fill was redistributed under the slides.

Nap cots in the PreK room do not have labels on the cots reflecting the current children using the cots during the inspection.

Based on evidence collected during this investigation, it was determined that the operation failed to ensure that a parent was contacted after discovering a child was displaying multiple symptoms of illness preventing the child from participating comfortably in activates.

Based on evidence collected during this investigation, it was determined that the operation staff failed to respond appropriately to a child's symptoms of illness. The child was seen in the classroom laying on the floor coughing, and observed to be what was described as burning, yet no temperature was taken or first aid given by staff.

During investigation, it was determined based on the video footage a child was separated from the group for more than 2 minutes, approximately 3 minutes, exceeding the one minute per year of the child?s age.

It is found a child was left unsupervised for 7 minutes in a classroom.

While reviewing the monthly infant feeding schedules, six of eight schedules were not updated within the last thirty days.

While reviewing traning records. Two of sixteen employee files did not include verification of Pre Service Training as required.

Two of ten children's files did not include a current immunization record.

During an investigation, it was determined that a caregiver knowingly allowed children to remain outside of visual and auditory awareness while attending to other duties in the classroom.

Based on the interviews conducted, a caregiver did not conduct a name to face while lining the children up inside the classroom and when arriving outside before the children start to play to ensure all children were accounted for. This resulted in a child being left in the classroom unsupervised for approximately 12 minutes. Based on the interviews conducted, five children between ages 18 months and 2 years old were left on the playground by a caregiver with another caregiver watching the children from a distance from a classroom in the doorway.

Based on the interviews conducted a parent was not notified immediately when a child was left in a classroom unsupervised. The parent was notified at pick-up.

The fire extinguishers in both vans used for transportation have not been serviced in the last year.

One child's file reviewed did not contain an immunization record.

At the time of inspection there were 11 children with one caregiver in the 18 to 24 month old classroom. This was corrected when a second caregiver joined the group.

Investigation information obtained from DFPS Child Care Licensing investigator determined there was a child injured while in care at the operation when a caregiver sprayed a "cleaning" solution in the children's faces.
